I hereby certify that I am able to walk at least 2 hours a day, that I am in good health, and that I undertake to fulfill my personal and collective responsibilities.*

I confirm my statements as being sincere and true and release Pur Azur from any liability in this regard. I also undertake to notify Pur Azur of any change in my health situation that may occur before my stay.*

The center declines all responsibility in the event of loss, damage or theft of money, etc.*

I declare that I understand that this is not a therapeutic center.*

I declare that I have read the contraindications on the Pur Azur website and that I am not affected by any of the following contraindications: pregnancy, breastfeeding, heavy drug treatments, psychological disorders and eating disorders, excessive thinness, insufficient vital force, cardiovascular disease, diabetes, stomach or duodenal ulcer and detachment of the retina.*

I understand that if I have any medical problems, or if I am taking medication, I must request a medical certificate of fitness for fasting from my doctor.*
